Abstract
Logistics companies are growing rapidly to accommodate diverse consumer needs. Rapid development requires risk management oversight in various areas of the company, including the warehouse, which is a crucial part of the operational process. Effective risk management in the warehouse area, serves to ensure the survival and growth of the company in a competitive market. PT ABCD's warehouse is facing operational problems due to the company's system that is not running as expected which is influenced by failures in internal and external management. This research is intended to identify and manage risks using the failure mode and effect analysis method approach. This research involved interviews with four resource persons, namely HRD, warehouse manager, accounting manager and marketing manager. In this study, a quantitative approach was used to convert potential failures and their impacts into numerical values to measure the risk priority number value, so as to identify the critical value of each risk indicator. The results showed that there were 9 risk indicators from 4 variables that had been determined. After analyzing these indicators in depth, 32 sub-indicators were identified to be evaluated based on severity, occurance and detection. There are 4 risk indicators that have RPN values above the predetermined critical values, namely supplier relations, inventory management, inbound management, and human resources.

Perkembangan yang pesat memerlukan pengawasan manajemen risiko di berbagai area perusahaan, termasuk gudang yang merupakan bagian krusial dalam proses operasional. Manajemen risiko yang efektif pada area gudang, berfungsi untuk memastikan kelangsungan dan pertumbuhan perusahaan di pasar yang kompetitif. Gudang PT. ABCD menghadapi masalah operasional karena sistem perusahaan yang tidak berjalan sesuai harapan yang dipengaruhi oleh kegagalan dalam manajemen internal dan eksternal. Penelitian ini dimaksudkan untuk mengidentifikasi serta mengelola risiko dengan pendekatan metode failure mode and effect analysis. Penelitian ini melibatkan wawancara dengan empat narasumber, yaitu HRD, manager warehouse, manager accounting dan manager marketing. Dalam penelitian ini, digunakan pendekatan kuantitatif untuk mengkonversi potensi kegagalan dan dampaknya menjadi nilai numerik guna mengukur nilai risk priority number, sehingga dapat mengidentifikasi nilai kritis dari setiap indikator risiko. Hasil penelitian menunjukkan adanya 9 indikator risiko dari 4 variabel yang telah ditentukan Setelah menganalisis indikator tersebut secara mendalam, teridentifikasi terdapat 32 sub-indikator yang akan dievaluasi berdasarkan severity, occurance dan detection. Terdapat 4 indikator risiko yang memiliki nilai RPN di atas nilai kritis yang telah ditetapkan, yaitu supplier relation, pengelolaan inventory, manajemen inbound, dan sumber daya manusia.
